Booking & Intake Forms

A healthier choice for a healthier you.

Online Booking

You can book appointments online anytime. Just click the "Book Appointment" button on our website to access our secure scheduling portal. Choose your service (e.g., individual or family therapy), select a date and time, and provide basic details. Once submitted, you'll receive a confirmation via email or text. Our online booking is available 24/7—schedule at your convenience without the need to call.

Phone Booking:

Prefer to talk to a person? No problem! During office hours, you can call us at 405-858-8656 to schedule an appointment. Our friendly front desk staff will help find an appointment time that fits your schedule, answer any preliminary questions, and guide you on next steps. If you’re not sure what type of service or which therapist is right for you, we can discuss your needs and match you with the appropriate professional on our team.

Intake Forms

Once you have booked your first appointment (either online or by phone), we’ll ask you to complete our new client intake forms. These forms gather important information such as your contact details, medical history, current concerns, and insurance/payment information. They also include our consent forms and privacy practices. We provide these forms digitally to make the process easy:

1

After scheduling, you’ll receive a secure link via email to fill out the intake forms online. You can complete them at your own pace from your computer, tablet, or smartphone.

2

If you prefer, you can also find the intake forms on our website under the “New Client” or Intake Forms section and fill them out through our secure portal.

3

All information you provide is confidential and protected. Our online forms are HIPAA-compliant to ensure your privacy.

4

Completing the intake forms before your first session helps your therapist prepare for your visit and ensures that the entire first session can be focused on you rather than paperwork.

Your First Appointment

What to expect...

After your forms are submitted, you’re all set! When you arrive (either in person or log in for a teletherapy session), we’ll review any important details from your intake and then dive into discussing what brings you to therapy and how we can help. The first session is a chance for your therapist to get to know you and for you to ask any questions about the therapy process. We know starting therapy can feel intimidating, but our goal is to make you as comfortable as possible.

Need to Reschedule or Cancel? Life happens – if you need to reschedule or cancel an appointment, please notify us at least 24 hours in advance. You can do this by calling our office or using the link in your appointment confirmation email. This allows us to offer that time slot to another client who may be waiting. We’ll work with you to find a new time that suits you best.

Most Popular Questions

We’ve compiled answers to some common questions you might have as a new or prospective client. If you don’t see your question here, feel free to reach out and ask!

We accept Medicaid (including Title 19/SoonerCare), Aetna, Humana, and most major private insurance plans. If you have Medicaid, Title 19, or SoonerCare, our services are fully covered and free of charge to you. For private insurance, we’ll check your benefits and let you know about any co-pay or deductible. (See our Insurance & Payment section for full details.) We also welcome private-pay clients.
MCS provides therapy for children, adolescents, and adults of all ages. Our team includes counselors experienced in working with young kids (typically ages 5 and up), teenagers, young adults, and older adults. Whether it’s a child struggling in school, a teen dealing with social pressures, or an adult facing life’s stresses, we have specialists who can help. We tailor our approach to be age-appropriate and relevant for each stage of life.

Yes! We offer secure teletherapy sessions via video call. If you prefer to attend therapy online or can’t make it to the office, simply let us know or select the telehealth option when booking. You’ll receive a link before your session to join a private video meeting with your therapist. Teletherapy at MCS is easy to use, confidential, and just as effective as in-person sessions for many issues. All you need is a quiet space and an internet-connected device (smartphone, tablet, or computer). Please note: at this time, our therapists can provide telehealth services to clients located within the states/regions we operate in (for licensing reasons). We’ll guide you through the setup process—many of our clients love the convenience of online therapy.
You can schedule an appointment by booking online through our website or by calling our office at 405-858-8656. Online booking lets you choose a therapist, service, and time slot that works for you, all in a few clicks. If you call, our staff will happily assist in finding the right therapist and time based on your needs. Whether online or by phone, scheduling is straightforward. If you’re a new client, we’ll also provide instructions for completing intake forms (see above) to get you set up. Current clients can use our online system or contact us directly to book follow-up sessions.
Absolutely. Confidentiality is one of the cornerstones of our practice. Everything you discuss with your therapist is private and protected by strict privacy laws (HIPAA) and ethical guidelines. MCS will not share your information with anyone without your written permission, except in a few rare circumstances where we are legally required to do so for safety reasons – for example, if there is an imminent risk of serious harm to you or someone else, or in cases of child/elder abuse that we’re mandated to report. Your comfort and trust are extremely important to us, so feel free to talk with your therapist about any concerns regarding privacy. We have robust measures in place (secure records, encrypted telehealth, etc.) to ensure your information remains confidential.
If you are experiencing a mental health crisis or any emergency (such as suicidal thoughts with intent, hallucinations, or feeling at risk of harming yourself or others), please seek immediate help – call 911 or go to the nearest emergency room. You can also contact the 988 Suicide & Crisis Lifeline by dialing 988 for 24/7 crisis counseling. MCS is an outpatient counseling provider and not equipped for emergency response. However, once you are safe, you can reach out to us for follow-up care and ongoing support. For urgent but non-life-threatening situations, let us know and we will do our best to schedule you as soon as possible. We also provide resources and after-hours voicemail on our phone line with instructions for crisis services. Your safety is our top priority. Always get immediate help first in an emergency, and we’ll be here to support you in your recovery after the crisis has passed.
